IMPORTANT HEALTH FORM FOR ALL COMPETITORS
Venue Requirements:
- Temperature Log (must be completed for three days prior to arrival on campus):
- Coggins: This horse must have proof of negative Coggins within 1 year prior to entering the UConn Storrs campus.
- Rabies: This horse must have been vaccinated for rabies within 1 year prior to entering the UConn Storrs campus.
- Eastern/Western/West Nile Virus/Tetanus needs to be given within 1 year prior to entering the UConn Storrs campus.
- Flu/Rhino Booster: This horse has been given a booster for Flu/Rhino within 6 months but no sooner than 14 days prior to entering the UConn Storrs campus.
The below form must be filled out and returned before unloading at the facility:
